Within the community, there are hidden economic disparities that exist. Carbon County has a vulnerable population with significant health needs. In recognizing these conditions and needs for change, the Healthy Communities Steering Committee developed PEAK (Providing Easily Accessible Care.)
PEAK is currently gathering data through a survey so that they can best assess the needs of the community and create a priority based off feedback. It is confidential and a great way to help our community move forward to a healthier community.
After analyzing the data the Healthy Community Steering Committee will develop a strategic plan that will provide resources for available health care and health care options, connecting health care providers for continuity and efficiency, and providing education for all parties to best utilize the resources.
